Adding a Constant Contact Universal Code

I need to add this universal code:
(edited it to keep it secure)
needs to be pasted before the body – I installed plugin called add header/footer
and tried to paste this into the header section that way – but it doesn’t seem to be working as it is not displaying the form on the contact pageIs there a different way for me to install this so that the form displays on the page?
